Thursday & Friday 4:30PM to Midnight and Saturday Noon to midnight.

Licensed social worker in state of Texas with BSW or MSW. Medical social work experience preferred. 1 yr acute medical hospital social work experience required

BSW or MSW will be accepted

Healthcare Management

Collaborates with case manager for medical management involvement by diagnosis, procedure, high utilization of high cost.
? Possesses knowledge of healthcare industry and terminology.
? Protects hospital from risk management issues.
? Demonstrates knowledge of healthcare industry delivery, and human anatomy and physiology to facilitate quality care.
? Complies with regulatory standards (JCAHO, EMTALA, TDI, HIPAA)
? Intercedes to avoid unnecessary diagnostic tests and procedures.
? Works to reduce Avoidable days and Denied days.
? Helps patients and families with governmental healthcare eligibility and benefits.
? Assesses for appropriate level of care after discharge (SNU, ICF, LTAC, Rehab, Hospice, home health, assisted living).

Community Resources and Support

? Demonstrates a breadth and depth of knowledge of community resources.
? Addresses issues related to patients non-compliant behaviors.
? Refers patients/families to emotional support resources.
? Reduces barriers to accessing healthcare and promoting health recovery.
? Develops and maintains knowledge of current legislative changes affecting healthcare practices.
? Addresses patient s vocational/ return to work needs, when needed.
? Collaborates with other healthcare professionals to promote health wellness and illness/injury education.
? Explores patient/family s current and projected financial resources for healthcare and essential needs (employment, SSDI, SSI, STD, LTD, FLMA, Work Comp).
? Pursues social change, particularly with and on behalf of vulnerable and oppressed individuals and groups of people.
? Acts on behalf of vulnerable individuals and takes appropriate actions to safeguard their interests and rights (child maltreatment, elderly/disabled maltreatment, domestic violence, incapacitated/incompetent)

? Knowledge of human behavior is used to assess coping skills.
? Documentation of Advanced Directives and discussion of palliative care is facilitated.
? Counseling therapeutic skills are employed to facilitate patient/ family coping.
? Interactions, evaluation, referrals and counseling are adjusted based on the patients age and developmental stage.
? Interactions, evaluation, referrals and counseling are adjusted based on the patients cultural, ethnic and spiritual influences.
? Interactions, evaluation, referrals and counseling are adjusted based on the patients financial, vocational and educational status.
? Theories, models and techniques are applied to the assessments and interventions and referrals in addressing psychiatric issues.
? Theories, models and techniques are applied to the assessments and interventions and referrals in addressing substance abuse issues.
? Listening and reflective skills are used to communicate genuineness and empathy and provides non-directive &/or directive feedback.
? Patients medical record reflects that care is transferred to internal or external resources to provide higher level or continuity of care.
