The core values of Community Health Center of Southeast Kansas (CHC/SEK) are dignity and stewardship. Each staff member is expected to perform their job duties in a way that preserves dignity for our patients and maintains good stewardship of the Center’s resources.
This position is responsible for a variety of care management activities with a focus on eliminating barriers for patients to access medical, dental, behavioral health services . This person will have a knowledge of available resources and programs within CHC/SEK and the community. This position is responsible for maintaining documentation that supports financial assistance and eligibility in special programs.
Applicants must possess a high school diploma or equivalent required. Any combination of education and work experience that would likely provide the required knowledge and abilities could be qualifying. Successful candidates must be able to communicate with others to effectively convey information and possess a social and cultural sensitivity appropriate to ethnically and economically diverse patient-and employee-base.
