We are looking to recruit dynamic Care Coordinators to join our client in assisting the Lead care coordinator in providing the correct care for their clients.
The successful candidate should have previously worked in the Health and Social Care Sector or have relative experience, Full Driving Licence and meet the following criteria:
Duties will include:
• Allocating staff with weekly work rotas & arranging cover for sickness/holidays. You will be directly responsible for the weekly rotas of care services
• Dealing with telephone calls from service users, their families & other healthcare professionals
• Assessing new clients and updating care plans
• Supporting and effectively managing senior care staff and care workers
• Help provide care when required on an emergency basis during mornings, evenings and weekends
• Having responsibility of the on call phone when required based on a rota
- Assess and discuss risks, reduce risks, share information and complete written report
- Adhere to all health and safety policies and the health and safety at work policy (1974)
- Report any hazards identified to the Managing Director or others and externally as required by policy and procedure.
- Ensure that CW complete incident accident reports
