Devereux’s Children’s Behavioral Health Services is currently seeing an Intensive Case Manager for Short-term Residential Services. The Intensive Case Manager acts as an advocate and provides problem resolution and team collaboration for clients in obtaining needed after care services for education, medical and mental health services. Works closely with primary therapist to ensure high quality service delivery.
Responsibilities include the following, but not limited to: Participates in pre-admissions screenings and weekly team meetings for all clients in the SRT program
Coordinates care and after care for individuals in mental health, medical, placement and related areas
Coordinates, monitors and reviews the provision of services to ensure individuals are receiving and participating in services on a continual basis. Links individual to appropriate mental health, medical, or other appointments providing transportation as needed.
Maintains direct contact via telephone and face to face with caseload and parent/guardian. Serves in liaison capacity communicating with the individual, the family/guardian, agency representatives and other stakeholders/advocates per agency guidelines.
Ensures the timely and accurate completion of related documentation to include service documentation, outcome reports, authorizations, releases, and assessments.
Assists in the hospitalization process as required.
Meets agency required productivity goals weekly. Is able to be independent in creating weekly schedules to allow enough appointments as well as needed paperwork time. Maintains a flexible work schedule to meet individual/family needs.

Education & Experience: BA/BS undergraduate degree in a Human Services discipline, experience in Case Management a plus. 1 year experience with children.
Physical Requirements: Significant amounts of walking, sitting, and standing. Some lifting and/or managing resistance required related to applying physical intervention techniques – upwards of 75-100 lbs. or more. Some repetitive wrist/hand movements, normal hearing and vision. Frequent use of computers and office equipment. Occasional driving of Devereux vehicles for business purposes. Ability to meet 100% mastery of Safe and Positive Approaches (SPA) techniques.
Work Environment: Regularly works inside areas that are adequately lighted and ventilated. Some fluctuations in temperature. Protective equipment – latex gloves, facial mask protective eyewear, gown. Possible exposure to blood-borne pathogens. Physical involvement in the exercise of SPA intervention techniques.
Core Curriculum: As required annually by licensing agencies and Devereux (organization and Center-specific modules).
