STAFF NURSEWard 4JPart-Time  (.8) D/N This position is located at the Minneapolis VA Health Care System in the Extended Care & Rehabilitation Service Line, unit 4J.  4J is a 18 bed CARF accredited acute rehabilitation and polytrauma unit.  Impairments most commonly treated on ward 4J are TBI, CVA, polytrauma, and amputee. It is organized to provide 24/7 skilled nursing care provided by a combination of RN’s, LPN and NA staff members.  Services address the full range of functional, physical, cognitive, psychosocial and leisure needs typically exhibited by individuals requiring acute rehabilitation. The program serves individuals 18 years and older.  The population served include veterans and active military personnel through the partnership with the DOD. The goal of nursing care and the interdisciplinary team on 4J is to treat patients and the prevention of complications associated with disease and disorder; performing diagnostic and therapeutic procedures; educating patient/family to improve on self-care abilities, health maintenance, and rehabilitation. Specialized training to provide rehabilitation nursing and behavioral management is provided during orientation. Duties include:- Provides age-appropriate and culturally and ethnically sensitive care to patients in the extended care and rehabilitation service line. – Maintains a safe environment, which may include patient on special or 1:1 supervision. – Educates patients about healthy practices and treatment modalities assuring continuity of care. – Coordinates care across settings and among caregivers. – Manages information and information systems. – Communicates effectively with all team members. – Utilizes technology specific to the work area. – Utilizes safe patient handling equipment per hospital policy. – Cleans RME equipment per standard operating procedures. – Works effectively within an interdisciplinary team. – Maintains current hospital and unit based competencies. – Responsible for wound care management which includes dressing changes, amputated stump care, and prosthetic training. – Performs routine tracheostomy care to include emergency management insertion and/or care of urinary drainage devices (condom cath, intermittent cath, Foley cath, suprapubic cath, and nephrostomy tubes). – Performs ostomy care. – Routine IV, PICC, and Central line care. – Ensures proper nutrition via oral and enteral feedings. – Works with patient goals toward independence. – Updates ITP information for team rounds. – Primary nursing model of care. – Ensures BLS training requirements are met every 2 years.
