Duties:
This position is for a Community Based Outpatient Clinic (CBOC) Senior Social Worker. One position is available at the St. Mary’s, FL CBOC, which is part of the North Florida/South Georgia Veterans Health System. The incumbent will work for the CBOC Chief Medical Officer. The Social Work Chief has professional oversight responsibility. Others duties includes: · Serves as an interdisciplinary team member and provide services for Veterans with primary care and/or mental health needs. · Must use a high level of skill in assessing and treating complex psychosocial and mental health needs of Veterans and their family members/caregivers, often during times when the Veteran/family members/caregivers are experiencing crisis. · Exercises mature professional judgment and use a wide range of clinical skills in independently providing clinical psychosocial, case management, and mental health services to Veterans and their families/caregivers. · Requires knowledge of a broad range of psychosocial, medical, and mental health issues. · Provides group and individual counseling/psychotherapy in either a face to face or telehealth setting, appropriate to level of licensure. · Coordinates care, develop and use VA and community resources to facilitate referrals, develop and implement treatment plans and provide accurate information to patients, family, staff, and other internal and external customers in a timely manner. · Works closely with an interdisciplinary team, it is important that the incumbent is a team player. · Provides coverage as needed for co-workers, which may include social work coverage in other parts of the System. · Complies with all policies/regulations of the Department of Veterans Affairs. · Attends and participate in administrative and clinical staff meetings. · Participates in the student training program, as well as, serve on facility, VISN, and national committees and workgroups.
