Under the direction of Home Health Program Director and the Supervising RN, the Client Services Coordinator assists in the daily operations of the Home Health office by answering the telephone and performing general office duties. The candidate for this position should possess excellent oral/ written communication and organizational skills. Qualifications
Performs General office duties, including, but not limited to the following: Opening/ Sorting mail; preparing/ sending outgoing mail; collecting Plan of Care documentation from doctors’ offices; ensuring client eligibility for various services; ordering adequate Inventory supplies under the supervision of the Supervising RN; maintaining neat and clean office area.
Responsible for entering Payroll data and Mileage Reimbursement reports on Personal Care; accepts Aide worksheets and compiles new Admission charts on patients and Aides to ensure that all records and forms necessary for the successful operation of Home Health services are submitted in a timely manner upon review and approval by the Supervising RN.
Coordinates the Nurse Aide schedules and times with adjoining counties if coordinating with two different counties; maintains client records to ensure timeliness of patient visits; Performs Quality Assurance Reviews, Discharge Audits, and ensures that Terminated files are completed; assures that all current Agency procedures are followed and most recent forms are utilized.
Responsible for ensuring that all appropriate paperwork is given to the CSA/ Nurse Aide, ensuring that the scheduled patients receive care; informing CSA/ Nurse Aide to contact RN so that they can be instructed of their duties as well as contacting the patient so they can be informed of the services to be rendered; notifying the RN of any calls from CSAs/ Nurse Aides regarding schedule changes or issues.
