Duties:
The Cancer Care Navigator Team (CCNT) Nurse Practitioner is responsible for leading the multidisciplinary CCNT, the multidisciplinary Hospice and Palliative Care Team and assumes the leadership role of facilitating end of life care throughout the organization and ensures compliance with associated directive, handbooks, and medical center memoranda. The incumbent works to improve care and coordination of treatment of eligible cancer patients throughout the facility and the surrounding community. The CCNT is responsible for development and continued effective functioning of the local CCNT while working collaboratively with other VISN 20 facilities’ CCNTs and the VISN 20 CCNT Program Manager to improve coordination of care for all VISN 20 cancer patients. Specific activities include but are not limited to: Serve as reference to PACT teams/Specialty teams
Point of contact for any work-up related questions from patient/family, staff (local and/or other VISN 20 facility), community providers
Inform patient/family of process, general disease education
Track suspicious imaging and pathology reports to conclusion
Ensure timely Tumor Board review, participate in review, and support communication of plans
Identify high-risk patients
Track all treatments according to care plan
Assure restaging occurs in a timely fashion
Assure treatment summary and recommendations are communicated to all involved services
Track all surveillance monitoring to ensure timely diagnostics
