Duties:
Duties *The Community Engagement & Response Team (CERT) social worker (MSW) will serve as a member of a multidisciplinary treatment team. *In addition, the social worker will provide direct care and treatment where indicated and as such functions as a treatment team member. The social worker should have good working knowledge in all evidence based psychotherapies and treatments related to substance use disorders and mental health disorders. *The Social Worker must have a thorough understanding of psycho-dynamics and use a wide range of treatment modalities including crisis interventions, assertive community treatment, individual case work, group work, patient education, motivational interviewing, supportive therapy, family therapy, discharge planning, knowledge of aftercare treatment resources, etc. The treatment modality shall be consistent with the needs, as well as, goals and objectives of each case. Individuals shall be involved with and participate in formulating and implementing their own recovery efforts insofar as they are capable. *The social worker obtains a thorough working knowledge of aftercare community recovery treatment resources and makes appropriate referrals. *The incumbent makes every effort to link patients to the appropriate resource and to implement the aftercare plan *The social worker serves as a liaison between inpatient setting, outpatient setting and community services. *The incumbent advises significant others about community recovery resources available to them (Al-Anon & NAMI) as well as informing aftercare facilities about VA programs and resources *The incumbent of this position will engage with veterans in their community of residence and provide intensive case management and stabilization as the primary duty. The incumbent may spend up to 80% of work time engaging in intensive community based outreach with high-risk veterans
Work Schedule: Monday – Friday from 8:00am to 4:30pm
