The LISW Therapist is responsible for providing therapy services to patients and their families as part of a multidisciplinary team. Practices in both inpatient and outpatient settings. Demonstrates ability to provide diagnostic impression. Develops, implements, reviews treatment plans and reviews. Demonstrates ability to provide and update clinical assessments. Communicates verbally and in writing with physicians and other providers. Completes psychosocial assessments. Facilitates individual, family, and group therapy. Provides case management, discharge planning while documenting within required time frames. Ability to effectively assess, plan, and implement therapeutic services in a multidisciplinary team setting. Knowledgeable regarding all populations with background in medical social work.
