The Birth Certificate Coordinator obtains correct information for birth certificate reporting and prepares the certificate in accordance with state Vital Statistics rules and regulations.
Responsibilities:
- Interviews obstetrical patients to obtain required information for birth certificate. Reviews the medical record of the mother and baby to accurately complete the health section of the certificate.
- Reviews information to ensure complete and accurate information is attached to the birth certificate. Informs mother and/or father of any necessary reporting and charge requirements in the completion or changes in birth certificate information after discharge. Provides patient with verification of birth.
- Enters data into the electronic birth registry system in accordance with state vital statistics rules and regulations for every newborn.
- Forwards accurate and complete birth certificates to county health department within required time frame. Flags all adoption recordings according to policy.
- Files medical records and maintains integrity of patient’s medical record.
- May prepare birth/death certificates for the State of Tennessee, in accordance with State regulations.
- Processes all requests for release of patient’s medical information in compliance with State, Federal, and other regulatory agencies guidelines. All other duties as assigned.

Qualifications Education:
- High School diploma or equivalent
Work Experience:
- One year of experience in an office environment required.
- Previous experience in the handling of patient health information and/or medical records is preferred.
