Work with hospice, home health or assisted living facilities and clinical teams to manage staffing request for LPN and CNA’s for Continuous and Transitional Care Cases. Ensure availability of LPN’s and CNA’s for staffing when calls come inManage administrative intake of hospice, home health or assisted living facility calls or the admission/discharge information post notification Manage the referrals process, processing incoming and outgoing referrals, and prior authorizations, including intake, notification and census rolesInitiate and follow-up with hospice, home health or assisted living facilities to initiate new business or follow-up on existing business calls/visits Review requests for adverse determination and approval notification. Assist with setting up documents and reports for staff Review, submitted to Director of Operations Handle resolution/inquiries from hospice, home health or assisted living facilities that may include: CNA and LPN staffing availability (time sensitive) Billing and Payment customer issues Customer material requests Physician Orders Requirements:
