Duties:
The Hospice and Palliative Care Social Worker whose primary responsibilities are to provide clinical psychosocial and case management services to eligible Veterans and their family members with serious or life limiting illness to meet biological, psychological, social, and environmental needs. The Social Worker coordinates, plans, implements, monitors, and evaluates the delivery of services within HPC program. The incumbent is responsible for the coordination of the Palliative Care Consult Team (PCCT) and provides supervision of volunteers working in the HPC program with Veterans/families. The incumbent monitors the efficiency, effectiveness, timeliness, and quality of HPC services provided. This includes monitoring and reporting quality indicators to the Chief of Home and Community Based Services prior to sending out. The incumbent may participate in other duties and projects as assigned by the Chief HCBS.Work Schedule:Monday through Friday, 8:00 .m. to 4:30 p.m.
