Duties:
The LPN/LVN is accountable to the Kenai/FBX/Juneau (respective area) CBOC Nurse Manager for the supervision and performance evaluation with consultation with the input from the Telehealth Coordinator. The LPN/LVN works under the direct supervision of an RN. Position Summary: Provides outpatient primary care and Telehealth services to patients who present with urgent/acute/chronic medical conditions; change in mental status, functional status and/or social decline. The LPN is responsible for observing, recording and reporting changes in the patient’s condition, as well as assisting in the development and revision of the patient-driven holistic life care plan. Completed work needs only a general review by an RN for appropriateness and conformity with license, policies and procedures. The LPN demonstrates active collaboration with the PACT Team and the Expanded Team members.  Duties of this position include, but are not limited to the following: ·         Makes observations and thoroughly documents care and the patient’s response. ·         Performs, reports and records assigned nursing care in the Primary Care Clinics, including, but not limited to: -Oxygen Therapy.-Medication administration to include IV medication of certified.-Clinical reminders with appropriate consults and alerts as indicated.-Check expiration dates of meds and supplies.-Delegated procedures and treatments.-Documentation.-Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy. Monitors patient during procure(s). ·         Performs Telehealth Clinical Technician (TCT) duties such as imaging, tele-retinal screening, tele-dermatology, tele-wound care and other Telehealth programing as technology becomes available. ·         Responsible for the patient site facilitation of the clinical, business, scheduling, and technical aspects for teal-time Telehealth applications.·         Provides patient and family health education, to include self-management, prevention and wellness.  Work Schedule: Monday through Friday 8:00 a.m. to 4:30 p.m. Functional Statement Title/#: Licensed Practical Nurse/000000
