Position Description:
Under the supervision of the Director of Clinical Services, the Assessor performs a variety of Clinical and Administrative work when it has been determined through an initial screening that the youth/adult has mental health or addictive disease concerns. The initial Behavioral Health Assessment is required within the first 4 days of service, with ongoing assessments completed as demanded by individual client need and/or by service policy. The Behavioral Health Assessment process consists of a face-to-face Comprehensive Clinical Assessment with the individual, family member/responsible caregiver(s), and others significant in the youth’s/adult’s life as well as interviews with collateral agencies/treatment providers/relevant individuals. This position includes data gathering and formulation of an Individual Resiliency Plan/Treatment Plan which are all needed before the provision of mental health services to children and Adolescents in need of treatment and support. The purpose of the Assessment process is to perform a formalized assessment in order to determine the youth’s problems, strengths, needs, abilities and preferences, to develop a social (extent of natural supports and community integration) and medical history, to determine functional level and degree of ability versus disability, and to develop or review collateral assessment information. The information gathered should support the determination of a differential diagnosis and assist in screening for/ruling-out potential co-occurring disorders. This position requires a broad knowledge of therapeutic processes and methodologies.
Duties and Responsibilities:
- Receives referrals of clients in need of mental health services. 2) Conduct assessments in office and out of office. 3) Travel to conduct assessments. 4) Schedules, conducts, and completes a face to face assessment of client’s needs within the specified time frame. 5) Ensures the Orientation Packet is completed thoroughly. 6) Completes and submits the LOCUS/CaLocus 7) Completes and submits the Child and Adolescent Functional Assessment Scale (CAFAS) 8) Completes documentation (Assessment/Treatment Plan BIRP note) and ensure they are S.M.A.R.T. 9) Assists the client and family with the development of the Individualized Resiliency Plan (IRP)/Treatment Plan. 10) Submits the IRP to APS or the appropriate CMO. 11) Monitors the authorization status of Individualized Resiliency Plans / Treatment Plans. 12) Corrects Individualized Resiliency Plans / Treatment Plans as specified by APS or CMOs. 13) Prints Authorized Individual Resiliency Plans /Treatment Plans. 14) Turns in the completed Assessment Process to Utilization Specialist. 15) Participates in various staff meetings and in-services and communicates with Intake Staff. 16) Meet with the Director of Clinical Services weekly to update him/her on any clinical and programmatic issues 17) Ensure that all MICP/Treatment plans are signed by teams after being printed from the APS system 18) Contact referring agency Probation Officer or Caseworker once referral is received and maintain collaboration with referring party. Verbally contact the referral source at least twice per month 19) Submit weekly/monthly logs to the referring Caseworker, School Official or Probation Officer for case updates on the required dates 20) Ensure that new referrals and existing clients are scheduled for both Psychological and Psychiatric evaluations within 5 days of services (also upon expiration of evaluations within 12 months ) 21) Ensure all documentation relevant to consumer’s treatment is documented in consumer’s records 22) Attend mandatory Treatment Team Meetings and Supervision weekly 23) Attend monthly chart review as scheduled by the Utilization Department 24) Follow all agency policies and procedures 25) Assessor will provide Director of Clinical Services or designee with weekly schedule of assessments as required. 26) Input client information into Sharenote 27) Plans and facilitates community outreach and education activities, and may serve as an SBZ Services representative at community organizations 28) Provides counseling, therapy, and/or psychotherapy to consumers and families on an as needed basis. 29) Create new charts and discharge client’s charts according to proper insurance. 30) Utilize SBZ Services audit tool when conducting chart audits. 31) Audit maintenance and ensure the integrity of charts for active and discharge clients. 32) Input initial assessment/treatment plan and ongoing goals into Sharenote. 33) Cross train for other positions. 34) Conduct Family Team Meetings and Child Care & Family Assessment with Department of Human Services as needed. 35) Assist with filing all paperwork. 36) Print MICPs, ongoing MICPs and discharge MICPs. 37) Input unit authorization into Sharenote and 60 day unit authorization into Sharenote for clients. 38) Assist with coverage of doctor appointments, doctor days and other office duties. 39 ) Ensure all clients are reassessed 30 days prior to their annual anniversary date, every year and thus forward 42) Completes weekly and monthly reports regarding assessments and follow-up with referrals source/family. 43) Other duties as assigned.
Minimum Qualifications:
- Contractor must have a Master’s degree in Social Work, Clinical Psychology, Counseling or a closely related field and actively working toward licensure.
- Experience completing assessments and Individual Resiliency Plans preferred.
- Demonstrated knowledge of Mental Health disorders and treatment methods.
- Ability to provide skilled treatment to clients with Social and Behavioral problems.
- Ability to establish and maintain working relationships with consumers, families, service providers and colleagues.

- Knowledge of clinical operations and procedures, Medicaid services, and Community Mental Health resources.
