Responsibilities:
Primary Purpose
Scans/files patient information and documentation to the appropriate location of the health information record. Prepares, assembles, files, maintains, and/or delivers/retrieves health information records. Processes release of health information requests. Performs quality control of health information records and scanned images to ensure appropriate information is included.
Major Responsibilities
Scans/files patient information and documentation to the appropriate location of the health information record.
Creates, assembles, files, and/or maintains health information records.
Retrieves health information records/documents from various departments and clinic areas, and delivers to appropriate areas as needed.
Responsible for purging and/or destroying health information records, and for combining/merging duplicate health information records.
Processes release of patient information and health information record requests for continuity of care.
Monitoring the health information records for deficiencies and delinquent dictation.
Performs quality audits on charts and quality checks of scanned images and makes corrections according to department policy.
Performs clerical functions which may include: answering telephone calls, copying, faxing, completing/processing forms, documenting for care management, distributing mail, and/or ordering/delivering department or facility supplies.

Knowledge, Skills & Abilities
Excellent communication, interpersonal, customer service, and organizational skills.Basic to intermediate computer skills including proficient keyboarding skills and experience in using Microsoft Office products, electronic mail and/or electronic health information records systems.Demonstrated ability to read numbers and use the color-coding system. Must have the ability to comprehend and follow instructions.Must have high attention to detail and accuracy.Ability to work independently with minimal supervision and make decisions with good judgment.Demonstrated strong problem solving and analytical skills.Ability to prioritize workload and work under pressure in a fast-paced environment with time constraints.

This is a 1.0 FTE position, Monday through Friday 8:00am-430pm.
