Duties:
RN (Staff) Albany Community Based Outpatient Clinic (CBOC) Uses the Nursing process and evidence-based practice to collaborate with the Teamlet member (family/caregiver, internal and community-based services involved in providing care to the patient) to develop the patient-driven holistic care plan for life


  • Collaborates with services internal and external to the VA to facilitate care transition in order to effectively meet the patient’s needs

  • Provides patient and family health education with a focus on self-management; prevention, and wellness, based on the patient’s goals

  • Demonstrates leadership by serving as an advocate for patients

  • Demonstrates professional nursing practice characterized by leadership in developing and implementing programs to improve patient care

  • Monitors the service plan to ensure quality, quantity, timeliness and effectiveness of service

  • Initiates and participates in system review of clinical programs

  • Acts as a consultant, mentor or preceptor in Primary Care CBOCs

  • Provides continuing education to all providers and staff in the CBOC clinics which ensures a delivery of quality care

    • Work Schedule: Monday-Friday, 0800 – 16:30; may require extended week day clinic coverage
