Preferred Candidates: The ideal candidate will have completed a residency, have board certification or equivalent clinical experience, anticoagulation certification, BCPS and/or BCACP, experience with anticoagulation clinic and patient care.The incumbent serves as a Licensed Clinical Pharmacy Specialist (CPS, Anticoagulation) outpatient provider with prescribing privileges as outlined in the individuals scope of practice in the Pharmacy Service at the Fresno VA Central California Healthcare System, CA. The primary responsibility of the Clinical Pharmacy Specialist is the provision of evidence-based pharmaceutical care services through both direct and indirect interaction with patients and providers. This pharmacist promotes and manages anticoagulation therapy based on current clinical knowledge that is consistent with policies established at the national, VISN, and local levels. Activities of the incumbent may include but are not limited to:
- Participation in drug use evaluation.
- Developing and collection of quality assurance data.
- Providing on-going quality assurance monitoring.
- Implementing actions to correct identified problems with all involved services.
- Be an active member of any focused review teams.
- Provide dosing recommendations and therapy guidance in the perioperative setting.
- Participation in the Medication Use Committee when necessary.
The CPS (Anticoagulation) is an active member of the anticoagulation treatment team and is directly involved in the design, implementation and monitoring of therapeutic drug plans related to anticoagulation. They are the primary provider to initiate, modify or discontinue patients medication therapy related to anticoagulation and manager of side-effects, drug interactions, and disease states affecting anticoagulation. The CPS (Anticoagulation) works closely with the medical staff, house staff, pharmacy staff, interns, residents, students and nursing service to promote the safe, economical, and rational use of drugs.
