United States of America: Youth Services Coordinator - Teen Pregnancy Prevention


Reporting to Project Director, the Youth Services Coordinator will ensure that youth enrolled in the CHS teen pregnancy prevention program who may need supplementary supports to benefit from positive youth development programming have timely and coordinated access to needed medical, social, or behavioral health services. The Youth Services Coordinator will solicit supportive services that bolster youths’ protective factors and reduce their risk factors.


The New Jersey-based Center for Human Services (CHS) office operates under the US-based Health, Education, and Training Programs division. CHS began serving the people of Cumberland County, NJ in 1988 in a manner that acknowledges and respects their cultural norms, values and beliefs, which has resulted in the community trusting CHS to deliver quality services.

Job Responsibilities:


  • Completing the intake process

  • Assessing the comprehensive needs of each youth, developing an individual service plan, as needed and advising the participant and his or her parent/caregiver about the findings of the assessment and services available to meet his or her needs identified in the assessment

  • Ensuring each participant signs an assent form and that his or her parent/caregiver signs a consent form

  • Implement each participant’s service plan and monitor service delivery by coordinating with direct service providers and the parents/caregivers

  • Contacting youth and parents/caregivers to conduct follow up assessments and determine if youths’ have additional needs and the outcomes of services received

  • Establishing and maintaining updated case files for each participant, complete with case notes and reports

  • Collaborating with the data coordinator to ensure that accurate information on each participant has been entered into the project databases. Participate in data audits as needed

  • Participate in periodic in-service trainings on youth development and adolescent sexuality

  • Work with the evaluator to conduct follow up surveys as scheduled

  • Contribute information to facilitate the writing of project reports

  • Perform, as assigned, other activities deemed appropriate by the Project Director, that will enhance participant services and achieve the project goals

Job Requirements:


  • Demonstrated experience working with sensitive and confidential information

  • Track record of effectively working directly with youth

  • 4 to 5 years of experience providing case management services to youth

  • Demonstrated understanding of youth development

  • A bachelor’s degree in Social Work, Nursing, or related discipline.

  • Fluency in English and Spanish is required; must be bilingual.

  • Familiarity with HIPPA, and other laws governing confidentially in healthcare

  • Exceptional verbal, interpersonal, and written communication skills

  • Ability to prioritize and handle multiple tasks under tight deadlines

  • Keen attention to detail

  • Strong time management and organizational skills; demonstrated ability to work well independently and in teams

  • Proficiency in the Microsoft Office Suite of program (Word, Excel, PowerPoint) and database entry and database entry

  • Ability to provide effective follow-up on individual activities in a fast-paced, multi-tasking environment

  • Track record of being able to produce work in a timely manner with flexibility when necessary to adapt to changing work conditions.

  • Ability to pass a criminal background check

  • Reliable transportation to travel to faith-, community-based, and youth-serving organizations throughout Cumberland County
