Integrated Care Therapist utilizes a variety of person centered, culturally responsive, whole-person, family-oriented treatment approaches that meet the needs of the diverse population of the primary care setting. Services provided may include: providing effective behavioral health therapy to clients to improve their functioning and to prevent a higher level of care, group and family therapy, social emotional group activities, skills groups, behavioral supports for health conditions, crisis management, behavioral health case management, care coordination and consultation.
