CP Rochester is interviewing for the following Direct Support positions:
Residential-Based Habilitators to assist individuals with daily living skills, achievement of life goals, and personal care in a group home environment. Part-Time and Full-Time evenings, weekends & overnight hours available .
Community-Based Caregivers to provide interactive activities and supervision for children and adults in their own home. Activities are interest based and include playing games, doing crafts, reading with individuals, creative play, etc.; caregivers are responsible for ensuring health and safety of the individual. Late afternoon, evening & weekend hours available.
Community-Based Habilitators to work with individuals in their home or community providing opportunities to learn and strengthen skills to increase independence, build supports, and strengthen/expand skills. Monday-Friday 3pm-9pm and Sat-Sun varying hours .
Respite Caregivers to provide afternoon care at CP Rochester’s Winton Road Campus and support participants in daily recreational activities. Caregivers assist individuals in exploring activities, engaging with peers, and provide personal care supports as needed. Part-Time Monday-Friday 2:30pm – 6:00pm ; Full Time (shared Day Hab position) 9a-5p M-F.
Day Program Caregivers to assist individuals in participating in group activities (based on interest), assist Habilitators in group facilitation, provide community based experiences, and ensure health, safety, and personal care needs of individuals are met. Part-Time Monday-Friday 9:00am – 2:30pm .
Day Program Habilitators to expand opportunities for community inclusion, facilitate group activities, support individualized goals, teach independent living skills, and ensure health, safety, and personal care needs of individuals are met. Habilitators complete daily documentation of goal progression. Full-Time Monday- Friday between the hours of 8:00am – 4:00pm.
Direct Support Position Requirements :
- A high school diploma or GED – Required
- A valid and clean New York State Driver License
- Excellent verbal & written communication skills
- Physical Requirements include: sitting, standing, walking, lifting up to 50 pounds.
- College coursework in a human service related field and/or 1-3 yrs. experience working with developmentally disabled population strongly preferred.
